LevelsRx Review: Is It Legit?

LevelsRx review: compounded semaglutide $249 a month, or $139 a month on a 12-month plan; its operator's FDA warning letter, pharmacies and cancel rules.

Last updated Oct 24, 2026
Quick answer

LevelsRx is an online GLP-1 program that prescribes both brand-name and compounded medication: compounded semaglutide and compounded tirzepatide. Plans start at $139 a month on a 12-month plan, $1,668 up front, or $249 month to month. We rate it 6.9/10 (Fair). It names the pharmacies that fill its prescriptions: RedRock Pharmacy, Health Warehouse, Precision Compounding Pharmacy and Triad Rx. It ships to all 50 states. Compounded drugs are not FDA-approved. LevelsRx has an FDA warning letter on file, so we do not recommend it.

LevelsRx is an online program that sells compounded semaglutide and tirzepatide, as weekly injections or an oral liquid, with OpenLoop Health clinicians doing the prescribing. It suits you only if you are ready to prepay a long plan and can live with strict cancellation rules.

We rate it 6.9 out of 10, tier Fair. It names its prescriber network and four pharmacies, and it charges no membership or consult fee, but its prices change from page to page and the company that runs it has an open FDA warning letter.

The catch is the price. Month to month, compounded semaglutide costs $249 and tirzepatide $349. The $139 and $179 rates are 12-month plans, billed $1,668 and $2,148 up front. The first month is lower, $88 or $188, through a one-time code.

It’s for you if
  • You want the prescriber network and the pharmacies named.
  • You want an oral liquid semaglutide as well as injections.
  • You are sure you will stay a year and can prepay $1,668 for semaglutide.
It’s not for you if
  • You want a low month-to-month price: semaglutide is $249 a month.
  • You want to cancel online or by email.
  • You want a refund if you stop early.
  • You want a program whose operator has no open FDA warning letter.
  • You don't want injections, or don't feel comfortable injecting yourself; the oral liquid is the only other option.

What is LevelsRx?

LevelsRx is run by Trinity HealthCare Supply, LLC, which trades as LevelsRx. Its terms call the operator "LevelsRx, LLC". The same LLC is registered in Florida with five other trade names, including altRx. altRx, the name on the company's FDA warning letter, is a separate GLP-1 site with its own prices and terms; it lists the same four pharmacies as LevelsRx.

LevelsRx is a platform, not a medical practice. It says it "does not provide medical services, does not employ healthcare providers, does not participate in clinical decisions, and does not practice medicine." OpenLoop Health clinicians decide who gets a prescription, and its footer says medical services are provided by Usable Health, PC and affiliated practices.

Here is how it goes. You answer an eligibility quiz, pick a plan and pay. An OpenLoop Health clinician then meets you "after checkout", by live video or by a written review depending on your state. If you are prescribed, a partner pharmacy fills the order; LevelsRx puts processing and delivery at about 4 to 5 business days.

The menu has compounded semaglutide and tirzepatide injections and an oral liposomal semaglutide liquid. A separate page shows Wegovy and Zepbound, and the FAQ says "Brand name medication pricing is based on insurance coverage." Compounded GLP-1s are prepared by a pharmacy to a prescription, and they are not FDA-approved; the safety section explains what that means.

Is LevelsRx legit?

The company that runs LevelsRx, Trinity HealthCare Supply, LLC, received an FDA warning letter dated June 8, 2026, under its altRx name. The letter is still open. LevelsRx does name who prescribes and who fills, which many programs do not.

The letter concerns altrx.com, not levelsrx.com. The FDA objected to claims that compounded semaglutide and tirzepatide were "clinically proven", and to product images labeled "altRx", which suggested altRx makes the drugs. LevelsRx's home page today says "Clinically Proven GLP-1 Treatments".

The pharmacies are named. The terms list RedRock Pharmacy (St. George, Utah), Health Warehouse (Florence, Kentucky), Precision Compounding Pharmacy (Bellmore, New York) and Triad Rx (Daphne, Alabama). LevelsRx says they are state-licensed 503A compounding pharmacies, which compound for a named patient on a prescription. It does not say which one fills your order.

The site shows a LegitScript badge. We could not confirm the certification ourselves, so we do not count it.

LevelsRx costs $249 a month for compounded semaglutide and $349 a month for tirzepatide on the month-to-month plan. The first month is $88 for semaglutide or $188 for tirzepatide, through a $161 code its plan list applies once. The oral semaglutide liquid is $269 a month, or $108 for the first month.

Longer plans cost less per month, and you pay for the whole term at once:

  • Semaglutide injection: $567 for 3 months ($189 a month), $954 for 6 months ($159), $1,668 for 12 months ($139).
  • Tirzepatide injection: $717 for 3 months ($239 a month), $1,314 for 6 months ($219), $2,148 for 12 months ($179).
  • Oral semaglutide: $657 for 3 months ($219 a month), $1,074 for 6 months ($179), $1,908 for 12 months ($159).

Each plan renews automatically at the end of its term. LevelsRx says there are no "hidden fees, membership costs, or lab charges", and shipping is included. A missed or late-cancelled appointment can cost a no-show fee, with no amount stated. The membership is self-pay and does not require insurance.

The prices you see first depend on the page you land on. The home page says semaglutide starts at $129 a month and tirzepatide at $169. Other LevelsRx landing pages say $68 and $109, or $179 and $209. None of these figures is a plan in the checkout list above, and the product pages show their plan options with no prices filled in. Check the total at checkout before you pay.

What to watch

Its operator has an open FDA warning letter, the advertised prices change from one landing page to the next, the low rates are 12-month plans paid up front, and you can cancel only by phone.

How does LevelsRx work?

The care can be a video call or a written review. LevelsRx says the consultation "may take place via live video or through an asynchronous provider review", depending on your provider and your state. The home page offers "Unlimited Video Calls With Clinicians" and "same-day" slots.

After you start, the FAQ says check-ins are "usually once a month in the beginning", and your provider may order lab tests. LevelsRx does not say when labs are needed or who pays for them. Refills ship once you complete a refill form.

Is LevelsRx safe? FDA status and warning letters

Trinity HealthCare Supply, LLC, the company behind LevelsRx, has an open FDA warning letter dated June 8, 2026, issued as "Trinity HealthCare Supply, LLC dba altRx". It covers website claims and labels for compounded semaglutide and tirzepatide on altrx.com. The FDA has not published a close-out.

One of its pharmacies has an FDA record. Triad Rx, at the Daphne, Alabama address LevelsRx lists, received an FDA warning letter on February 5, 2019, for making sterile drugs under insanitary conditions. The FDA closed out the letter on May 3, 2023.

On July 31, 2026, the FDA issued a Form 483, a list of inspection observations, to Red Rock Pharmacy's site in Springville, Utah. LevelsRx lists a RedRock Pharmacy site in St. George, Utah, and does not say which site fills its orders.

The bigger question with any compounded program is the drug itself. Compounded GLP-1s are prepared by a pharmacy to a prescription. They are not FDA-approved: the FDA doesn't review their safety, effectiveness or quality before they're sold, and they are not the same as Wegovy or Zepbound. Formulas can differ from one pharmacy to the next, and some add ingredients such as vitamin B12. Compounding pharmacies are licensed and inspected by state boards of pharmacy; outsourcing facilities (503B) also register with the FDA and are inspected by it. The FDA can issue warning letters when either breaks rules on safety or sterility. LevelsRx's own pages say its compounded drugs "are NOT FDA approved".

FDA warning letterJun 8, 2026No closeout published
What the website claimed

The FDA says the company’s own marketing or labeling was misleading — for example a claim about safety, results or approval, or a label implying the company makes a drug a pharmacy compounds. This is about how the drug was presented, not about the medicine a patient received.

“FDA observed that your website offers compounded drug products, including semaglutide and tirzepatide products. …”

What does an FDA letter mean?

A warning letter asks a company to fix something. It is not a recall, it does not suspend a license, and it is the FDA’s position rather than a court’s finding.

How to cancel LevelsRx

Cancellation
Cancel anytime
Money-back guarantee
A full refund if a clinician finds you ineligible before care begins. Otherwise fees are not refunded, and medication cannot be returned once it ships. A 6-month weight-loss guarantee has strict conditions.

You cancel by phone. The terms say: "To cancel your LevelsRx subscription, you must contact our customer support team by phone. Cancellations cannot be processed via email, chat, or website submission." The request has to reach LevelsRx at least 7 days before your next billing date. The terms of use, last updated in January 2026, describe canceling by email instead. Ask LevelsRx to confirm the cancellation date in writing.

The refund rules are short. You get a full refund for the billing cycle if a provider finds you medically ineligible before care begins. Otherwise, "ALL FEES ARE NON-REFUNDABLE", including if you cancel mid-cycle or stop treatment. Medication can't be returned or refunded once it is dispensed or shipped.

The home page promises: "Hit your target with us, or we refund 100% of your money." The terms set the conditions. The refund covers medication fees only, after 6 months, if you lose less than 20% of your starting weight. You must also attend 80% of weekly physician consultations, follow a dietitian meal plan with "100% adherence", and send a monthly scale photo. Your final weight has to be measured "at LevelsRx", where home scales are not accepted.

The trap is the long plan. At $139 a month, semaglutide means paying $1,668 up front, and fees are not refunded if you stop. Prepay only if you are sure you will stay on the drug that long. If you have side effects, talk to your provider.

What customers say about LevelsRx

LevelsRx has a 2.7 TrustScore on Trustpilot from 8 reviews. Five are one-star, all posted since July 2026, and three are five-star, from March 2026. LevelsRx has not replied to any of the negative reviews.

The one-star reviews describe an order still unshipped 10 days after ordering, a tirzepatide vial that arrived warm and cloudy, text messages to people who never signed up, and repeated calls after canceling. The five-star reviews praise the support staff and fast delivery. The home page shows the five-star reviews under "Great 4.2 | 6 Reviews".

LevelsRx's weight-loss figures, such as "94.6% success rate", are self-reported by members, and its patient photos are labeled "Inspired by real patient success stories in similar Telehealth programs". That is marketing, not results you can expect. This site collects no independent reviews of its own yet.

How we scored LevelsRx

Our score combines seven checks. Each one counts for a fixed share of the total, listed below. Commission is not an input.

  • Price transparency: 8.2/10 (counts for 20% of the total score). The full plan list is public and there is no membership or consult fee. We take a point off because seven LevelsRx pages advertise different starting prices that match none of its plans, and the product pages show no prices.
  • Cost and value: 4/10 (counts for 15% of the total score). We compare month-to-month prices: $249 for semaglutide ranks 11 of 16 programs that sell both compounded and brand-name drugs, against a median of $204.50. The longer plans are cheaper but are not in this comparison.
  • Medication and pharmacy traceability: 7.8/10 (counts for 15% of the total score). Four pharmacies named with their type, but not which one fills your order.
  • Clinical access and oversight: 7.2/10 (counts for 15% of the total score). A named prescriber network and monthly check-ins at first, but no stated lab policy.
  • Safety and enforcement: 5.5/10 (counts for 20% of the total score). An open FDA warning letter to its operator, and a 2019 letter, closed out in 2023, to one of its pharmacies. We take a further half point off because the home page still uses the "clinically proven" wording the FDA objected to.
  • Terms, commitment and refunds: 9/10 (counts for 10% of the total score). Cancel anytime, and the refund rule is stated. We take a point off because canceling is by phone only, with 7 days' notice, and the guarantee the home page advertises has conditions few people could meet.
  • Availability and fulfillment: 8.8/10 (counts for 5% of the total score). All 50 states, shipping included and a stated delivery time.

This is the first score for LevelsRx. Confidence is low: it is a new program, and its advertised prices change from one page to the next. Three of the seven checks carry an editor's adjustment, explained above; together they lower the total by 0.4. The method is on our methodology page.
