About this data set

What it measures
The US list price (WAC) of each GLP-1 product beside the price a person with no insurance pays in cash.
Units
List price: US dollars per package, as the maker publishes it. A pen or vial package covers 28 days; a tablet package holds 30 tablets. Cash price: US dollars per month (one fill), as the program states it.
Coverage
9 products: 7 brand-name semaglutide and tirzepatide products (Wegovy, Ozempic, Rybelsus, Zepbound, Mounjaro) and 2 compounded products. United States only.

August 27, 2026. Compounded prices: August 5, 2026. Each row states its own check date.
Method
We read each price from the maker’s cash program page, a maker announcement, or the checkout of a program in our register. Full method below.

List price against cash price

The percentage is (list − cash) ÷ list, rounded to the nearest whole number. Where the cash price is tiered by dose, we use the midpoint of the tiers.

Methodology

Definitions

List price (WAC). Wholesale acquisition cost: the price the maker publishes to wholesalers, per package, before discounts and rebates. It is not what a pharmacy pays, not what an insurer pays after rebates, and not a cash price.

Cash price. What a person with no insurance pays for one month through the maker’s own channel, as the program states it on its page.

Compounded price. The lowest entry price we verified at checkout across the compounded programs in our program register. Compounded drugs are not FDA-approved.

Inclusion rules

The index includes every brand-name semaglutide and tirzepatide product sold in the United States: Wegovy (pen and tablets), Ozempic, Rybelsus, Zepbound (vials and KwikPen) and Mounjaro. It includes compounded semaglutide and compounded tirzepatide as one row each. A price goes in only when we can read it on a public page or at a program checkout. Where a maker does not publish a WAC or a cash price, the cell says so. We do not estimate a missing price.

Update cadence

We re-check cash prices against the live program pages and compounded prices against the program checkouts. Each row states the date of its last check. The last full check of maker prices was August 27, 2026. The last full check of compounded prices was August 5, 2026.

Limitations

  • Units differ by column. WAC is per package (28 days for pens and vials, 30 tablets for tablets). Cash is per month. Compare the two only for products where one package is one month.
  • Many cash prices are tiered by dose. A range gives the lowest and highest tier. The spread table uses the midpoint.
  • Some cash prices have conditions: new-patient prices, refill windows, dose tiers. The note column states them.
  • Compounded prices are entry prices. The price at higher doses is usually higher, and it changes from program to program.
  • The index does not include insurance copays, net prices after rebates, or Medicare and Medicaid prices. For those, use the cost calculator.
  • The index does not include NADAC yet. NADAC is the next column we plan to add.
  • Prices change. Every figure is true on its check date only.
Questions

Questions about the index

WAC anchors a rebate-driven system: insurers negotiate large confidential discounts off it, so manufacturers keep it high. Cash programs bypass the middlemen entirely, which is why the same pen can be 74% cheaper bought direct. Judge affordability by the cash column, never the list column.

Wholesale acquisition cost — the manufacturer’s published price to wholesalers, before any discounts or rebates. It is not what pharmacies pay (that is closer to NADAC), not what insurers pay net of rebates, and not what cash programs charge. It survives mostly as a headline number.

Cash-program prices are re-checked against the live program pages, compounded entry prices against the checkouts of the tracked programs (last full pass August 5, 2026), and every row stores its source URL and as-of date. The weekly CMS NADAC feed joins the index next from the data warehouse.
