Savings cards require commercial insurance — they cannot be used on Medicare, Medicaid or Tricare (federal anti-kickback statute). Compounded prices are verified program entry prices, not manufacturer programs.

How this calculator works

Prices come from the manufacturers' own published cash programs — NovoCare, LillyDirect and TrumpRx — plus CMS NADAC acquisition data as context, and program prices verified weekly against live checkouts. Each row stores the source URL and an as-of date, shown beside the number.

The math is deliberately simple: monthly cost is the pathway's published price for a 28–30 day fill; the 12-month figure is that price × 12 with no discounting. Cost per percent of body weight lost divides the 12-month total by the trial-anchored loss at month 12 — 13.7% for semaglutide 2.4 mg (STEP 1) and 19.2% for tirzepatide (SURMOUNT-1) on our plateau curve.

Known limits: savings-card terms change yearly and cap annual benefits; intro rates (Wegovy's $199 first two fills, Zepbound's dose-tiered $299–449) depend on staying inside program rules; Medicaid varies by state. Where a figure is pending re-verification it is flagged in the row note.

Cost questions, answered directly

Almost nobody pays list. Manufacturer cash programs sell the same pens and vials for $149–$449 a month, savings cards cap commercial copays near $25, and telehealth programs sell compounded semaglutide from $69. List price mostly matters as the fallback when you leave a program’s rules.

Cash-pay telehealth programs fit your inputs

At your price point the verified compounded programs undercut every brand pathway. The register lists each program’s entry price beside its safety checks.

This calculator explains published prices and program terms; it never replaces your prescription, your plan documents or advice from a licensed clinician. NADAC is what pharmacies pay to acquire a drug, not what you will pay at a counter. Figures marked [VERIFY] are pending re-verification against primary sources.
