Who can get Zepbound?

Zepbound is approved for adults with obesity, or adults with overweight and at least one weight-related condition, to reduce excess weight and keep it off.1 It is also approved to treat moderate to severe obstructive sleep apnea in adults with obesity.1 The pivotal weight trial enrolled adults with a BMI of 30 or more, or 27 to under 30 with a condition such as high blood pressure, high cholesterol, sleep apnea or heart disease.1

How to get tirzepatide, step by step

  1. Get a prescription. See your own doctor or a licensed telehealth clinician. Zepbound cannot be bought legally without one.48
  2. Choose the form. Zepbound comes as a single-dose pen, single-dose vial, multi-dose vial or 4-dose KwikPen, each in six strengths from 2.5 mg to 15 mg.1
  3. Choose how to pay. Use insurance with the savings card if your plan covers it, or pay Lilly's self-pay price if it does not.2
  4. Choose the pharmacy. Send the prescription to LillyDirect Pharmacy or, for the KwikPen and pens, another pharmacy.3
  5. Refill on time. The lower self-pay price on 7.5 mg and up holds only if you refill within 45 days.2

You can get a Zepbound prescription online from a licensed telehealth clinician, who will review your history, weight and health conditions before prescribing. The clinician then sends the prescription electronically to a pharmacy. For LillyDirect, Lilly says the prescription is routed to LillyDirect Self Pay Pharmacy Solutions through the provider's electronic health record system.4

Telehealth programs differ in visit fees, membership costs and which tirzepatide they prescribe. Some prescribe brand-name Zepbound filled at LillyDirect; others sell compounded tirzepatide. Can you get Zepbound without a doctor's prescription?

No. Zepbound is a prescription drug, and a valid prescription is required whether or not you use insurance.4 The FDA warns that websites selling GLP-1 drugs without a prescription, or as "research" products, may sell counterfeit or harmful products.8

How to get Zepbound from LillyDirect

LillyDirect is Lilly's online pharmacy channel. Once your prescriber sends a Zepbound prescription there, LillyDirect fills it at Lilly's self-pay price with free home delivery or pharmacy pickup.3 Lilly and Walmart added in-store pickup at Walmart pharmacies in late 2025 for prescriptions placed through LillyDirect.4

Vials vs pens via LillyDirect

The Zepbound single-dose vial is available only through LillyDirect Pharmacy.3 It holds one weekly dose, and you draw it up with a syringe. If your commercial plan covers Zepbound, the savings card can bring the cost to as little as $25 for up to a 3-month supply, with savings capped at $100 a month.2 Many plans require prior authorization, where your prescriber documents that you meet the plan's criteria. Mounjaro is the same drug as Zepbound, tirzepatide, but it is approved for type 2 diabetes, not weight management.7 You get it the same way, with a prescription from a clinician in person or online. Compounded tirzepatide: the alternative

Compounded tirzepatide is sold by many telehealth programs, often below the brand's price, but it is not FDA-approved.
