How to Inject Zepbound (Tirzepatide) With a Pen, KwikPen or Vial
Learn the Zepbound injection sites, the best place to inject, and how to use each form. Covers time out of the fridge and travel, from Lilly's guide.
Zepbound comes in three forms that Lilly currently supports: a single-dose pen, a 4-dose KwikPen and a single-dose vial.8 Each one is injected a little differently. Everything below comes from the FDA-approved prescribing information and Lilly's Instructions for Use, which your prescriber or pharmacist should walk through with you before the first dose.1
Where to inject Zepbound
Inject Zepbound under the skin of your stomach (abdomen) or thigh, or have another person inject it into the back of your upper arm.1 It is injected under the skin (subcutaneously) only.2
Three rules apply to every site:
- Rotate each week. You can use the same body area, but choose a different spot within it each time.2
- Stay away from the belly button. The KwikPen instructions specify at least 2 inches from it.3
- Skip damaged skin. Do not inject where skin is tender, bruised, scaly, hard, scarred, pitted, thickened or lumpy.4 Rotating sites lowers the risk of these skin changes.4
Where to inject tirzepatide and Mounjaro
The sites are the same for every tirzepatide product. Mounjaro's label also lists the abdomen, the thigh, or the back of the upper arm when someone else gives the injection, with site rotation each dose.5 Do not use Zepbound with Mounjaro or any other tirzepatide or GLP-1 product.1
What is the best place to inject Zepbound?
No site gives better results: the label reports similar drug levels whether tirzepatide is injected in the abdomen, thigh or upper arm.1 So the best place is the one you can reach comfortably and rotate reliably.
The abdomen and thigh are the two sites you can reach on your own.2 The instructions reserve the back of the upper arm for injections given by another person.2 Your prescriber can help you choose.2
How to use the Zepbound single-dose pen
The single-dose pen is an autoinjector: place it flat on the skin, unlock it, press the purple button and hold for up to 10 seconds until the second click.2 It holds one dose and a hidden needle.2
- Prepare. Take the pen out of the refrigerator. Check the label for the right medicine, dose and expiry date. The liquid should be colorless to slightly yellow, not cloudy, frozen or full of particles. Wash your hands.2
- Choose a site on the abdomen or thigh, or the back of the upper arm if someone else is injecting.2
- Pull off the gray base cap while the pen is still locked. Throw the cap away and do not put it back on.2
- Place the clear base flat on your skin, then unlock by turning the lock ring.2
- Press and hold the purple button for up to 10 seconds. The first click means the injection started; the second click means it is complete.2
- Check the gray plunger. If it is visible, the full dose was delivered.2
- Dispose of the pen in an FDA-cleared sharps container.2
The instructions say air bubbles are normal and that you do not need to warm the pen to room temperature.2 A drop of liquid or blood on the skin afterward is also normal; press gauze on it without rubbing.2 If you drop the pen on a hard surface, do not use it, because it has glass parts.2
How to use the Zepbound KwikPen
The KwikPen holds 4 weekly doses: attach a new pen needle, prime it, turn the dose knob until it stops, then push and hold for 5 seconds.3 Pen needles are sold separately.3
- Prepare. Wash your hands, pull off the pen cap and check the name, strength, expiry and appearance of the medicine. Wipe the red inner seal with an alcohol swab.3
- Attach a new pen needle for every injection. Keep the outer needle shield and throw away the inner one.3
- Prime before each weekly dose. Turn the knob until you hear 2 clicks and the prime line shows. Point the needle up, tap out air, then push the knob in and count to 5. A drop at the needle tip means the pen is primed.3
- Dial your dose by turning the knob all the way until it stops and the full-dose icon shows. Do not count clicks.3
- Inject. Insert the needle, push the knob in until it stops and slowly count to 5 before removing it.3
- Remove the needle using the outer shield, put it in a sharps container, and replace the pen cap.3
After 4 doses, throw the pen away even if medicine remains, and do not move leftover medicine into a syringe.3 Never share a KwikPen, even with a new needle, because of the risk of passing on blood-borne infections.1
How to inject Zepbound with a syringe (vials)
With a single-dose vial, you draw 0.5 mL into a new syringe and inject it under the skin, holding the needle in for at least 5 seconds.4 The label advises a syringe that can measure the dose, for example a 1 mL syringe, and a new syringe and needle every time.1
- Prepare. Check the vial label and the liquid's appearance, wash your hands, pull off the plastic cap and wipe the rubber stopper with an alcohol swab.4
- Draw in air. Pull the plunger back to the 0.5 mL line, push the needle through the stopper and inject the air into the vial.4
- Draw the dose. Turn the vial and syringe upside down, pull past 0.5 mL, tap out bubbles, then push back to exactly 0.5 mL.4
- Inject. Insert the needle at the chosen site, push the plunger and wait at least 5 seconds before pulling out.4
- Dispose. Do not recap the needle. Put the syringe in a sharps container and throw away the opened vial, even if liquid is left.4
The label also lists a multi-dose vial with four 0.6 mL doses.1 Its measurements differ from the single-dose vial, so follow the instructions for the vial you were given. Does Zepbound need to be refrigerated?
Yes. Store unused Zepbound in the refrigerator at 36°F to 46°F (2°C to 8°C), in its original carton to protect it from light.1 Never freeze it, and do not use it if it has been frozen.1
How long can Zepbound be out of the fridge?
Single-dose pens and vials can be kept unrefrigerated, at up to 86°F (30°C), for a total of 21 days.1 After 21 days at room temperature, throw them away.1
The 4-dose KwikPen and multi-dose vial follow different rules:1
- Unopened: refrigerated, they last until the expiry date. Kept at room temperature (up to 86°F), throw them away after 30 days.
- Opened: store in the fridge or at room temperature (up to 86°F). Throw away after 30 days at room temperature, 30 days after first use, or after 4 weekly doses, whichever comes first.
Keep every presentation away from heat and light.1
How to travel with Zepbound
The label allows up to 21 days unrefrigerated for single-dose pens and vials, and 30 days for the KwikPen and multi-dose vial, as long as they stay at or below 86°F (30°C).1 On a long trip, plan around the total days out of the fridge and keep the carton out of heat and light.1
At US airports, the TSA allows medically necessary liquids in reasonable quantities in carry-on bags; declare them to officers for inspection.6 Unused syringes are allowed when they come with injectable medication, and must also be declared.7 TSA recommends, but does not require, keeping medications labeled.7 The final call rests with the officer at the checkpoint.6
FAQ
How do I know my Zepbound pen worked? The gray plunger is visible when the dose is complete.2 If you cannot see it, contact Lilly at 1-800-545-5979.2
What time of day should I inject Zepbound? Any time of day, with or without meals, on the same day each week.1 You can change the day if at least 3 days (72 hours) pass between doses.1
What if I miss a dose? Take it within 4 days (96 hours) of the missed day. If more than 4 days have passed, skip it and take the next dose on your usual day.1 Our missed dose tool works out the timing.
Should I pinch the skin? The vial instructions say your prescriber should tell you whether to pinch the skin.4 The pen instructions only ask you to place the clear base flat against the skin.2
Can I reuse a syringe or pen needle? No. Use a new needle for every injection to prevent infection and blocked needles, and never share needles, syringes or pens.43
