Every version of the card shares the same baseline eligibility. You must have a prescription for an FDA-approved use, live in the United States or Puerto Rico, be 18 or older, and not be enrolled in any state, federal or government-funded healthcare program, which Lilly spells out as including Medicaid, Medicare, Medicare Part D, Medicare Advantage, Medigap, DoD, VA and TRICARE.1 Lilly states it can end or change the program at any time without notice, and that the program is not insurance.1

The $25 Zepbound coupon: commercial insurance that covers Zepbound

If your commercial plan covers the Zepbound single-dose pen, the card can bring a 1-month, 2-month or 3-month fill down to as little as $25.1 The $25 figure is a floor, not a guarantee, because the card has caps:

  • up to $100 off a 1-month fill, $200 off a 2-month fill, or $300 off a 3-month fill;1
  • up to $1,300 off per calendar year;1
  • up to 13 fills per calendar year.1

Lilly defines one month as 28 days and up to four single-dose pens.1 In practice, whether you reach $25 depends on your plan's copay or coinsurance. If your share of the cost is more than $125 for a 1-month fill, the $100 cap means you pay the difference. Once the $1,300 annual cap is used up, the card stops reducing your price until the next calendar year.1

Coverage itself is the harder step. Zepbound coupon with no insurance coverage

If you have commercial insurance that does not cover Zepbound, or no insurance at all, the savings card does not get you to $25. Instead you pay Lilly's self-pay price. As of October 2026 Lilly lists these monthly prices for the Zepbound KwikPen for single-patient use:1

The $449 price on 7.5 mg and above is a purchase offer that applies automatically on your first fill of those doses, and it only continues if you complete each refill within 45 days of receiving the previous one.1 Miss that window and Lilly's regular price applies, which is $499 for 7.5 mg and $699 for 10 mg, 12.5 mg and 15 mg.1 A month here is 28 days and one KwikPen.1

Two other details matter. For people with commercial insurance that does not cover Zepbound, the KwikPen card can be used for up to 11 fills per calendar year.1 And if your commercial plan does not cover the single-dose pen, the card's floor for that device is $499 a month, which is more than the KwikPen or vial self-pay price for every dose.1

The federal TrumpRx site lists the same $299 starting price for Zepbound against a list price of $1,087.5 That listing does not change Lilly's dose ladder above.

Zepbound now comes in three presentations, and the price you pay depends on which one is prescribed.12

Lilly states that the self-pay prices apply to "the Zepbound KwikPen or vial, not Zepbound single-dose Pen."1 A self-pay month is one KwikPen or four vials.1 Lilly's provider page lists the same vial ladder, starting at $299 for 2.5 mg, $399 for 5 mg and $449 for 7.5 mg to 15 mg, with the same 45-day refill rule, and says single-dose vials are dispensed only through LillyDirect Pharmacy.2

So "LillyDirect Zepbound cost" and "Zepbound vial vs pen" come down to the same answer: as of October 2026, a vial and a KwikPen cost the same per month at each dose for self-pay patients, while the single-dose autoinjector is the device tied to the $25 commercial card.12 Which one suits you is a clinical and practical decision for you and your prescriber.

Can Medicare patients use the Zepbound savings card?

Mounjaro coupon and savings card in 2026

The Mounjaro savings card works like the Zepbound card but with higher caps, and it requires commercial insurance in both of its forms.3 Mounjaro is tirzepatide, the same molecule as Zepbound, approved for type 2 diabetes, and the card requires a prescription for an approved use consistent with the FDA label.3

So the "Mounjaro $25 coupon 2026" is real, but only for people whose commercial plan covers Mounjaro.
