Oak Longevity Review: Is It Legit?
Oak Longevity review: compounded semaglutide and tirzepatide from $130/mo, whether it names its pharmacy, FDA warning letters and complaints.
Oak Longevity is an online clinic for weight loss, sexual health and longevity care. It suits people who want compounded semaglutide or tirzepatide at a low monthly price, with no video call and no membership fee.
We rate it 8.4 out of 10, tier Good. It ranks 2 of 15 on month-to-month prices, every fee is $0, and the FDA record is clean.
The catch: the drug prices appear only after the medical quiz, and Oak does not name its pharmacies.
No FDA warning letter on file for Oak Longevity as of Aug 13, 2026, in the FDA’s published warning letter list.

What is Oak Longevity?
Oak Longevity is a telehealth brand that sells prescription care for weight, sexual health and longevity, delivered to your door. Weight loss is its main line.
Here is how it goes. You pick a treatment plan and fill in a short medical questionnaire. A US-licensed provider reviews it online and prescribes only when it is medically appropriate. No video call is needed. The prescription goes electronically to one of Oak's contracted pharmacies. The drug ships to your door, and refills continue on the same plan.
The weight-loss menu has two drugs, both compounded and both weekly injections. Compounded semaglutide is the molecule in Wegovy and Ozempic. Compounded tirzepatide is the molecule in Zepbound and Mounjaro. To understand the drugs themselves, read our pages on semaglutide and tirzepatide.
Compounded means a pharmacy makes the drug to a prescription. It is not an FDA-approved product, and that is the trade for the low price. Oak serves all 50 states.
Is Oak Longevity legit?
Mostly yes. Oak is a real telehealth brand with US-licensed providers, a stated prescribing process and no FDA warning letter. The gap is the pharmacy: Oak sends prescriptions to contracted pharmacies it does not name, so you cannot check who makes the drug.
Oak also does not say what type of pharmacy it uses. With no names, there is nothing to match against the FDA register we read. For a compounded program, where the pharmacy makes the drug itself, that matters.
The home page does not say plainly that the weight-loss drugs are compounded. Each drug page names the molecule and the form. Ask Oak for the pharmacy name before your first order, and look it up on your state board's site.
How much does Oak Longevity cost?
Compounded semaglutide costs $130 a month on a monthly plan, with the drug, the provider and shipping included. There is no membership fee and no consultation fee.
Compounded tirzepatide is $199 a month. The code OAKNEW50 takes $50 off your first month. The home page advertises weight loss "from $119/mo", below the $130 monthly rate we saw. Its terms describe 12-month plans at a lower monthly rate.
You see these prices only after the medical quiz. No public page lists the price of each drug. Check your price again at each refill. Oak does not bill insurance. Three months of semaglutide at the monthly rate total $390, with no fees added.
That puts Oak at rank 2 of the 15 programs in its group on month-to-month prices. The median there is $199 a month. For scale, a pharmacy pays $871.12 for a Wegovy 1.7 mg pen package, effective September 23, 2026. That is an acquisition cost for the brand-name drug, not what a patient pays.

How does Oak Longevity work?
The care runs online, with no video call. You fill in a health form, and a board-certified provider reviews it, often the same day. Approval can come within hours.
After that you get free access to the Oak Care Team and free health coaching. Your provider can adjust your dose during treatment. The terms describe a TeleMed consultation with one of the medical professionals Oak contracts with. They add: "If you have taken a lab test through the lab partners, you may discuss the results with the medical Provider you selected."
Oak does not state whether you need labs before you start, or how often follow-ups happen. Ask the care team both questions before you pay.
Is Oak Longevity safe? FDA status and warning letters
Oak Longevity has no FDA warning letter and no open flag on its record. Its record is clean. There is nothing against a pharmacy either, because none is named.
The safety question with a compounded program is the drug itself. A compounded GLP-1 is made by a pharmacy, and the FDA does not review it the way it reviews Wegovy or Zepbound. Oak asks you to complete a medical questionnaire, and a provider prescribes only when it is medically appropriate.
For side effects, doses and who should not take these drugs, read our semaglutide and tirzepatide pages, which quote the FDA labels.
How to cancel Oak Longevity
On a monthly plan you can cancel at any time, with no minimum term. Cancel before your next order goes to the pharmacy. Once a provider has prescribed and the order has gone to the pharmacy, you cannot cancel it and you get no refund.
The trap is the 12-month plan. Leave it early and Oak charges back the discount for the months you used. The terms give an example: "on a $209/month 12-month plan when the month-to-month price is $299, cancelling after 6 months of treatment results in a charge of $540 (6 × $90)."
So take a longer plan only if you are sure you will stay.

What customers say about Oak Longevity
We found no patient testimonial on the Oak pages we read, so there are no quote cards on this page. The only voice on the site is the brand's own. This site collects no independent reviews either.
That leaves you with little to go on from other patients. Oak is a newer platform, and independent reviews are still thin. Read the prices, the plan lengths and the refund rule above for what you will pay and what you sign. If you look for Oak Longevity reviews elsewhere, check what they say about the price after the quiz and the 12-month plan before you commit.
How we scored Oak Longevity
Price transparency scores 7.67 at a 20% weight: every fee is $0 and stated, but the drug prices sit behind the quiz. Cost and value scores 10 at a 15% weight: rank 2 of 15 on month-to-month prices, no fee load. Traceability scores 4.6 at a 15% weight: no pharmacy named, and compounding is not stated on the home page. Clinical care scores 7.2 at a 15% weight: same-day review, no lab policy.
Safety scores 10 at a 20% weight: no letter, no flag, a clean record. Terms score 10 at a 10% weight: monthly plans cancel anytime and the refund rule is stated. Availability scores 9 at a 5% weight: all 50 states and free shipping.
Our fresh computation gives 8.3, a move of 0.1. That is under our 0.3 threshold, so the published 8.4 stands until a later crawl confirms it. Since our last check, traceability fell from 5.8 to 4.6 after the home page changed. The weights are on our methodology page.
