That alert, issued on July 26, 2024, described patients who gave themselves 5 to 20 times the intended dose.1 One patient told to inject 5 units drew 50 units instead.1 One provider prescribed 25 units when the intended 0.25 mg was 5 units, and another prescribed 20 units instead of 2, giving three patients 10 times the intended dose.1 Reported effects included nausea, vomiting, abdominal pain, fainting, headache, dehydration, acute pancreatitis and gallstones, and some patients were hospitalized.1

The semaglutide units-to-mg formula

Divide the units by 100 to get mL, then multiply by the concentration in mg/mL.1

  • Units to mg: mg = (units ÷ 100) × mg/mL
  • mg to units: units = (mg ÷ mg/mL) × 100
  • Units to mL: mL = units ÷ 100

How brand-name Wegovy and Ozempic are measured

Wegovy and Ozempic pens are dosed in milligrams, with no unit markings to convert.23 Each product has a fixed volume per dose, which gives each strength its own concentration.

Wegovy single-dose pens hold 0.25 mg, 0.5 mg or 1 mg in 0.5 mL, and 1.7 mg, 2.4 mg or 7.2 mg (Wegovy HD) in 0.75 mL.2 The Wegovy FlexTouch pen holds 9.6 mg in 3 mL (3.2 mg/mL) and delivers four 2.4 mg doses.2 Ozempic pens hold 2 mg/3 mL (0.68 mg/mL), 4 mg/3 mL (1.34 mg/mL) or 8 mg/3 mL (2.68 mg/mL).3

Compounded semaglutide: what the calculator can't check

Compounded semaglutide is not FDA-approved, so its concentration, salt form and syringe instructions have not been reviewed by the FDA.4 The agency says it has no information on whether the salt forms some compounders use behave like the active ingredient in approved drugs.4 The FDA determined on February 21, 2025 that the semaglutide injection shortage is resolved.5

Before drawing a dose, four checks matter:

  1. Read the mg/mL on your own vial label. Concentrations vary between pharmacies and between refills.
  2. Write the dose in both mg and units, and confirm both with the dispensing pharmacist.
  3. Use the syringe you were given. The FDA asks providers and compounders to supply the right syringe for the dose and teach patients to measure it.1
  4. Throw multi-dose vials away 28 days after first use, even if medication remains.4

Is 50 units of semaglutide a lot? It depends on the concentration. 50 units is 0.5 mL, which is 1.25 mg at 2.5 mg/mL but 2.5 mg at 5 mg/mL.1 Wegovy's usual weight maintenance dose is 2.4 mg weekly, and Ozempic's maximum is 2 mg.23

Is 40 units or 80 units of semaglutide too much? At 5 mg/mL, 40 units is 2 mg and 80 units is 4 mg.1 4 mg is above every Ozempic dose and every Wegovy dose except Wegovy HD 7.2 mg.23 Ask your prescriber before injecting any volume you are unsure of.

How many mL is 40 units of semaglutide? 40 units on a U-100 syringe is 0.4 mL, whatever the concentration.1
