The FDA approved Zepbound in November 2023 for chronic weight management in adults with a BMI of 30 or more, or 27 or more with at least one weight-related condition.3 The August 2026 label also lists moderate to severe obstructive sleep apnea in adults with obesity, the use a Part D plan may cover.4

Does Medicare cover Zepbound?

Medicare covers Zepbound (tirzepatide) two ways. For sleep apnea, a Part D plan may cover the drug, because Part D covers GLP-1s for approved uses other than weight loss, per KFF's May 2026 analysis.1 For weight loss, Part D cannot cover any drug by law; the only Medicare route is the GLP-1 Bridge. The Bridge covers Zepbound KwikPen at a $50 copay a month, per Medicare.gov as read in September 2026, and runs from July 1, 2026 through December 31, 2027, per KFF and CMS's July 13, 2026 page.126

The Bridge was first set to end on December 31, 2026. On April 21, 2026, CMS announced that it would not run the BALANCE Model in Medicare in 2027 and would instead extend the Bridge through the end of 2027.1 CMS's own Bridge page, last modified July 13, 2026, gives July 1, 2026 to December 31, 2027 as the dates.6 The Bridge covers Zepbound KwikPen only; it does not cover single-dose Zepbound vials or single-dose pens.2

People with type 2 diabetes, moderate to severe sleep apnea, or fatty liver disease are not eligible for the Bridge, because Medicare.gov says their drug plan should cover GLP-1s for those conditions.2 CMS tells Part D plans in its 2026 guidance that Zepbound prescribed for sleep apnea does not qualify for the Bridge, even when the drug is off the plan's formulary.7

Will Zepbound be covered by insurance in 2026?

For Medicare beneficiaries who qualify, yes, through a new program. The Medicare GLP-1 Bridge started on July 1, 2026 and covers Zepbound (tirzepatide) KwikPen at a $50 copay a month, per the CMS page updated July 13, 2026 and the Medicare.gov page read in September 2026.62 The Bridge does not stop at the end of 2026: CMS lists December 31, 2027 as the last day.6 For Medicaid, coverage shrank: 13 states covered GLP-1s for obesity as of January 2026, down from 16 in 2025, per KFF.1

People who already get a GLP-1 through Part D for type 2 diabetes, heart risk or sleep apnea keep getting it through their Part D plan, not through the Bridge, which covers GLP-1s only for obesity, per KFF's May 2026 analysis.1
