Is metformin a GLP‑1?

No. Metformin belongs to a different drug class and does not act on the GLP-1 receptor.1 Its label says metformin decreases glucose production in the liver, decreases glucose absorption from the intestine and improves insulin sensitivity by increasing glucose uptake.1 The label also states that its mechanisms differ from other classes of oral diabetes drugs.1

Is metformin like Ozempic?

Ozempic vs metformin

Ozempic and metformin both treat type 2 diabetes, but Ozempic is a weekly GLP-1 injection with heart and kidney indications, while metformin is a daily tablet approved for blood sugar control.21 Ozempic is also approved to reduce the risk of major cardiovascular events in adults with type 2 diabetes and heart disease, and to reduce kidney decline in those with chronic kidney disease.2

On weight, the two differ sharply. In a 29-week placebo-controlled study in the Glucophage label, weight changed by −1.4 lb on metformin and −2.4 lb on placebo, a difference that was not statistically significant.1 The label says weight on metformin "tended to remain stable or even decrease somewhat."1 In a 56-week Ozempic trial in which 94% of patients also took metformin, average weight fell 4.2 kg on Ozempic 0.5 mg and 5.5 kg on Ozempic 1 mg, against 1.7 kg on sitagliptin.2

Metformin vs semaglutide for weight loss

Semaglutide produces much larger average weight loss than metformin in trials, and only semaglutide (as Wegovy) is approved for weight reduction.31

  • Wegovy 2.4 mg, adults without diabetes: 14.9% average loss at 68 weeks, against 2.4% on placebo.3
  • Wegovy 2.4 mg, adults with type 2 diabetes: 9.6% at 68 weeks, against 3.4% on placebo.3
  • Metformin, Diabetes Prevention Program: after 1 year, 28.5% of the metformin group had lost at least 5% of body weight, against 13.4% on placebo.6

For comparison, 83.5% of adults on Wegovy 2.4 mg in the main weight trial lost at least 5%.3 The studies used different populations and lengths, so the numbers show scale rather than a head-to-head result.

Metformin's long-term record is real. In the Diabetes Prevention Program, metformin cut new cases of type 2 diabetes by 31% against placebo over an average of 2.8 years, while an intensive lifestyle program cut them by 58%.5 Among people who lost at least 5% in the first year, those originally on metformin kept off an average of 6.2% between years 6 and 15.6

Metformin vs Mounjaro

Mounjaro (tirzepatide) is a weekly GIP and GLP-1 injection for type 2 diabetes, and its main trials added it on top of metformin rather than replacing it.4 In SURPASS-2, adults already on metformin who added Mounjaro lost 7.6 kg at 5 mg, 9.3 kg at 10 mg and 11.2 kg at 15 mg over 40 weeks, against 5.7 kg with semaglutide 1 mg.4 HbA1c fell by 2.0 to 2.3 points on Mounjaro, against 1.9 on semaglutide.4 See Mounjaro dosage and Mounjaro vs Ozempic.

Can you take metformin and Ozempic together?

Yes, the combination is common: Ozempic, Wegovy and Mounjaro were all studied in people already taking metformin, and their labels do not call for a metformin dose change.234 The Ozempic label lists metformin as needing no dose adjustment when given with semaglutide.2 The Wegovy label reports no clinically significant change in metformin levels with semaglutide.3

The labels' low-blood-sugar warnings are about insulin and sulfonylureas, not metformin.24 Metformin alone does not usually cause hypoglycemia, though the risk rises when it is combined with sulfonylureas or insulin.1

Two practical cautions come from the labels:

  • Stomach side effects can stack. Diarrhea affected 53.2% of patients on metformin monotherapy, against 11.7% on placebo, and nausea and diarrhea are among the most common GLP-1 side effects.13
  • Dehydration matters. The metformin Medication Guide tells patients to contact their doctor if they have severe vomiting, diarrhea or fever, because dehydration may mean pausing metformin.1

Side effects: metformin vs GLP‑1 drugs
