What is the starting dose of Mounjaro?

The starting dose of Mounjaro is 2.5 mg injected under the skin once a week.1 It is also the lowest dose made. The label tells prescribers to follow the starting dose and escalation schedule to reduce the risk of stomach side effects.1

After at least 4 weeks on a dose, the prescriber can raise Mounjaro by 2.5 mg if more blood-sugar control is needed.1 The schedule does not require every patient to climb to 15 mg.1

When to increase the Mounjaro dose for weight loss

For Mounjaro, the label ties dose increases to blood-sugar control, not to weight loss: a step up is considered after at least 4 weeks on the current dose if more glycemic control is needed.1 Mounjaro has no weight-loss dosing instructions because it is not approved for weight loss.1 Zepbound, the tirzepatide brand approved for weight reduction, uses the same 2.5 mg steps every 4 weeks.2

Can I increase the Mounjaro dose after 2 weeks?

No, not under the label: each dose step requires at least 4 weeks on the current dose.1 Moving faster raises the risk of the stomach side effects the slow schedule is meant to limit.1

What dose of Mounjaro is best for weight loss?

Mounjaro is not approved for weight loss, but in its diabetes trials weight fell more on higher doses: −6.3 kg on 5 mg, −7.0 kg on 10 mg and −7.8 kg on 15 mg at 40 weeks, against −1.0 kg on placebo.1 Those figures come from SURPASS-1, a 40-week trial of Mounjaro alone in adults with type 2 diabetes.1

In adults with obesity and no diabetes, tirzepatide sold as Zepbound produced larger losses. In the SURMOUNT-1 trial, average weight fell 15.0% on 5 mg, 19.5% on 10 mg and 20.9% on 15 mg after 72 weeks, against 3.1% on placebo.32 Higher doses also brought more stomach side effects, which is why the label leaves the maintenance dose to the prescriber.1

How to take Mounjaro

Mounjaro is injected under the skin once a week, on the same schedule, at any time of day, with or without meals.1 The label lists three injection areas: the abdomen, the thigh, or the back of the upper arm when another person gives the shot.1 Rotate the injection site with each dose.1

Best time of day to take Mounjaro

What happens if you miss a dose of Mounjaro?

Take the missed dose as soon as possible within 4 days (96 hours); if more than 4 days have passed, skip it and take the next dose on your usual day.1 In either case, go back to your regular weekly schedule afterward.1 Our missed dose tool works out the timing.

How long does it take for Mounjaro to work?

Can you take half a dose of Mounjaro?

The label does not describe splitting Mounjaro doses, and the KwikPen is built to deliver only fixed doses.4 The KwikPen instructions say to turn the dose knob all the way until it stops for each weekly dose, and not to count clicks.4 After 4 doses, the pen is thrown away, including leftover medicine, which should not be injected or moved into a syringe.4
