The week 12 and week 24 figures come from post hoc analyses of trial data, which are exploratory. They describe groups, not what any one person will see.

When does Zepbound start working on appetite?

Zepbound starts working on appetite within the first weeks. In a 6-week trial of 114 adults without diabetes, tirzepatide cut energy intake at an all-you-can-eat lunch by about 525 kcal compared with placebo at week 3.3 The same trial found lower overall appetite, fewer food cravings and less perceived hunger with tirzepatide than with placebo.3

The label explains part of the early effect. Tirzepatide slows how fast the stomach empties, and that delay is largest after the first dose and shrinks over time.1 That is why the first weeks can feel very different from month six.

How long does it take for Zepbound to suppress appetite?

For most trial participants, appetite changes appeared within weeks, as measured at week 3 in the phase 1 trial.3 The size of the effect varies from person to person, and the starting dose of 2.5 mg is not meant to be a treatment dose.1

Why the first month on Zepbound can feel slow

The first 4 weeks are on 2.5 mg, a dose the label calls a starting dose, not a maintenance dose.1 The dose then steps up by 2.5 mg after at least 4 weeks on each dose.1 At the fastest pace, a person reaches 15 mg in week 21.1

Two things drive the slow start:

  • Dose. In the main trial, weight loss rose with dose: 15.0% on 5 mg, 19.5% on 10 mg and 20.9% on 15 mg at week 72.1
  • Time. Steady blood levels take 4 weeks of weekly dosing to build.1

How fast does Zepbound work for most people?

In SURMOUNT-1, 82% of participants who took their doses as scheduled lost at least 5% of their body weight by week 12.4 The analysis included 1,545 people treated with tirzepatide who received at least 75% of their assigned doses.4

The other 18% were labeled late responders.4 They were not non-responders:

  • 70% of late responders reached 5% weight loss by week 24.4
  • 90% reached 5% by week 72.4
  • The mean time to 5% weight loss for late responders was 24.8 weeks.4

A second analysis used week 8 as the cutoff and found that both early and slower responders reached clinically meaningful weight reduction by week 72, although early responders lost more.5 Its authors noted the timing and severity of stomach side effects were similar between the groups.5

How long does tirzepatide take to work? Mounjaro and compounded versions

Tirzepatide behaves the same way whether it is sold as Zepbound or Mounjaro: steady levels after 4 weeks of weekly dosing and a half-life of about 5 days.2 Mounjaro also starts at 2.5 mg and rises in 2.5 mg steps.2

Compounded tirzepatide is a different case. Does Zepbound work for everyone?

No. In the main trial, 9.1% of people on the 15 mg dose did not reach 5% weight loss by week 72.1 The rates of reaching at least 5% loss were 85.1% on 5 mg, 88.9% on 10 mg and 90.9% on 15 mg, against 34.5% on placebo.1

Results were lower in adults with type 2 diabetes.
