What to expect after the first dose of Zepbound

After the first Zepbound injection, blood levels peak in about a day, and the drug's slowing of stomach emptying is at its strongest after that first dose.1 The label gives a median time to peak concentration of 24 hours, with a range of 8 to 72 hours.1 It says the delay in gastric emptying is largest after the first dose and diminishes over time.1

What people may notice in week one:13

  • Fullness and less appetite. Tirzepatide decreases calorie intake, likely by acting on appetite.1
  • Stomach effects. Nausea, diarrhea, constipation, vomiting and indigestion are the most common reactions.1
  • Injection site reactions. Redness, itching or bruising occurred in 6% to 8% of people on Zepbound, against 2% on placebo.1
  • Allergic-type reactions. Immediate reactions within a day of a dose occurred in 2.1% of people on Zepbound, against 0.4% on placebo.1

When do Zepbound side effects start?

Stomach side effects can appear from the first dose, and the Zepbound label says most nausea, vomiting and diarrhea happened during dose escalation and decreased over time.1 The 2025 joint nutrition advisory says side effects are more likely in the first weeks of treatment and after a dose increase, and tend to decrease with a stable dose.3 It notes nausea often appears in the morning or after long stretches without food.3

In the weight-loss trials, 56% of people on each Zepbound dose had a gastrointestinal reaction, against 30% on placebo.1 Most people who stopped Zepbound because of side effects did so in the first few months, mainly for gastrointestinal reasons.1

How long does it take for Zepbound to suppress appetite?

The label does not give a number of days, but it shows the pieces: Zepbound reaches peak levels about 24 hours after a dose, slows stomach emptying most after the first dose, and reduces calorie intake by acting on appetite.1 The label also says GIP and GLP-1 receptors are found in brain areas involved in appetite regulation.1

Both labels raise the dose every 4 weeks, and the gastrointestinal side effects cluster around those increases.12 The table below follows the fastest label schedule; your prescriber may hold you at a dose longer.

What to expect when increasing your Wegovy dose

Nausea or other stomach effects may return after an increase; the Wegovy label says these reactions were reported most frequently during dose escalation.2 In the adult weight-loss trials, 44% of people on Wegovy injection reported nausea, 30% diarrhea and 25% vomiting, against 16%, 16% and 6% on placebo.2

The label gives a specific option for a hard step: if a dose is not tolerated during escalation, the prescriber can consider delaying the next increase by 4 weeks.2 In the trials, 6.8% of people on Wegovy 2.4 mg stopped because of side effects, against 3.2% on placebo, most often because of nausea, vomiting or diarrhea.2

The same pattern applies to Zepbound. Its label says that if a maintenance dose is not tolerated, a lower maintenance dose can be considered.1

When does Wegovy start working?

Wegovy starts acting after the first injection, but 0.25 mg is a starting dose and the label's recommended maintenance dose of 2.4 mg is reached in week 17.2 Semaglutide reaches peak concentration 1 to 3 days after an injection and has a half-life of about 1 week.2 The label says semaglutide reduces calorie intake, likely by affecting appetite.2

Warning signs in the first weeks

The labels list symptoms that need prompt medical attention, not just patience:124

  • Severe stomach pain that will not go away, with or without vomiting, which can be a sign of pancreatitis.14
  • Persistent vomiting or diarrhea, because dehydration can lead to kidney injury; the label advises avoiding fluid depletion.1
  • Signs of a serious allergic reaction, such as swelling of the face, lips or throat, or trouble breathing.14
  • Pain in the upper stomach, fever or yellowing of the skin or eyes, which can point to gallbladder problems.4
