Wegovy Pill vs Injection: Effectiveness, Doses, Cost and How to Take It
The Wegovy pill is daily oral semaglutide 25 mg. Compare it with the weekly injection on weight loss, side effects and cost, and learn how to take it.
The Wegovy pill (oral semaglutide 25 mg, taken once a day) produced weight loss close to the weekly Wegovy injection in trials, and it costs less at self-pay prices, but it must be taken every morning on an empty stomach.14 The FDA approved the Wegovy pill on December 22, 2025, as the first oral GLP-1 for weight management.2 This guide compares the Wegovy pill vs injection on results, side effects, doses and cost, and explains how to take the tablet, using the FDA label and the trial behind it.
Is the Wegovy pill as effective as the injection?
The Wegovy pill is close to the injection, but the two were never tested against each other in one trial. On the FDA label, adults taking Wegovy 25 mg tablets lost an average of 13.6% of body weight at 64 weeks, against 2.4% on placebo.1 Adults taking the Wegovy 2.4 mg injection lost an average of 14.9% at 68 weeks, against 2.4% on placebo.1
Novo Nordisk reports a higher figure for the pill, 16.6%, which estimates weight loss in people who kept taking the tablet as directed.2 The label's 13.6% counts everyone randomized, including people who stopped.1 Compare like with like: 13.6% (pill) against 14.9% (injection) on the label's method.
Blood levels explain why the results are similar. In people without type 2 diabetes, average semaglutide levels on the 25 mg tablet daily and the 2.4 mg injection weekly were similar.1 In people with type 2 diabetes, levels on the tablet were lower, and the label says some may get too little drug from the pill.1
The injection also has a higher dose. Wegovy HD 7.2 mg is approved for people who tolerate 2.4 mg for at least 4 weeks and need more weight reduction.1 The pill has no dose above 25 mg.1 For the full injection comparison against tirzepatide, see Wegovy vs Zepbound.
What did the OASIS 4 trial show?
OASIS 4 is the trial behind the Wegovy pill's weight-loss approval. It randomized 307 adults with obesity, or overweight plus a weight-related condition and no diabetes, to oral semaglutide 25 mg or placebo for 64 weeks.3 On the label, 76.3% of people on the tablet lost at least 5% of their weight, 47% lost at least 15% and 27.9% lost at least 20%.1 The placebo figures were 31.3%, 5.5% and 3.1%.1
The trial was published in the New England Journal of Medicine in 2025.3 The pill is also approved to reduce the risk of major cardiovascular events in adults with established cardiovascular disease and obesity or overweight.1
Wegovy pill vs injection side effects
The Wegovy pill and injection share the same side-effect profile, led by stomach symptoms. In OASIS 4, gastrointestinal side effects occurred in 74% of people on the tablet and 42% on placebo, and were mostly mild to moderate and temporary.3 The label says the types and frequency of common reactions with the tablet were similar to those seen with the 2.4 mg injection.1 With the injection, nausea affected 44%, diarrhea 30%, vomiting 24% and constipation 24%.1
Side effects led 6.9% of people on the tablet to stop for good, against 5.9% on placebo.1 Both forms carry the same boxed warning about thyroid C-cell tumors.1 For the full list, see Wegovy side effects.
What are the doses of the Wegovy pill?
The Wegovy pill comes in four strengths: 1.5 mg, 4 mg, 9 mg and 25 mg.1 Each step lasts 30 days.1
The schedule comes from the label, which recommends 25 mg as the maintenance dose.1 The gradual increase is there to reduce stomach side effects.1 For the injection schedule, see Wegovy dosage.
Can I stay on a low dose of the Wegovy pill?
The label does not list a lower maintenance dose for the pill. Its recommended maintenance dose is 25 mg.1 If a step is hard to tolerate, the label says a prescriber can consider delaying the next increase.1 If 25 mg is not tolerated, the label suggests considering a switch to the Wegovy 1.7 mg injection rather than staying on a lower tablet.1 Any decision to stay at 4 mg or 9 mg is a prescriber's off-label call.
How to take the Wegovy pill
Take one Wegovy tablet each morning on an empty stomach with up to 4 ounces of plain water.1 Then wait at least 30 minutes before eating, drinking anything else or taking other oral medicines.1 The label adds four rules:1
- Do not take it with any liquid other than water.
- Swallow the tablet whole. Do not split, crush, chew or dissolve it.
- Do not take more than one tablet a day.
- If you miss a dose, skip it and take the next one the following day.
These rules exist because absorption of oral semaglutide depends on an empty stomach and little water.1 The injection has none of them: it can be given at any time of day, with or without meals.1
What is the best time of day to take the Wegovy pill?
The label specifies the morning, on an empty stomach, before food, drink or other pills.1
Can I take the Wegovy pill at night?
The FDA label says to take the Wegovy pill in the morning, so taking it at night is not how it was approved.1 The empty-stomach and 30-minute rules are what make the tablet absorb, and a bedtime dose after an evening meal would not meet them.1 Ask your prescriber before changing the time. If a fasting routine is a problem, a weekly injection or Foundayo, a pill with no food or water rules, may suit better.
How much is the Wegovy pill?
The Wegovy pill costs $149 a month at 1.5 mg, $199 a month at 4 mg and $299 a month at 9 mg and 25 mg at Novo Nordisk's self-pay price, checked October 7, 2026.4 A month is one bottle of 30 tablets.5 The Wegovy pen costs $349 a month for 0.25 mg to 2.4 mg and $399 for Wegovy HD 7.2 mg, so the pill is $50 a month cheaper at maintenance.4
With commercial insurance that covers Wegovy, the savings card can bring the cost to as little as $25 a month, with savings capped at $100 a month.4 People on an eligible Medicare Part D plan may pay $50 a month for any dose of the pen or pill through the Medicare GLP-1 Bridge, which runs from July 1, 2026, to December 31, 2027.56 Live prices are on the Wegovy cost page, and our cheapest Wegovy ranking compares telehealth programs that prescribe it.
Which is better, the Wegovy pill or the injection?
Neither form is better for everyone. The injection has slightly larger average weight loss on the label and a higher dose option, and it is taken once a week at any time.1 The pill avoids needles and costs less at self-pay prices, but it demands a daily fasting routine.14 People with type 2 diabetes may absorb less semaglutide from the pill.1 Switching is possible: the label allows moving from the 2.4 mg injection to the 25 mg tablet one week after the last injection, and from the tablet back to the injection the day after the last tablet.1
