What Is Foundayo? Lilly's Orforglipron Pill, Explained
Foundayo costs $149 to $299 a month at Lilly's self-pay prices. Learn what orforglipron is, how to take it, its trial results, and how it compares.
Foundayo is orforglipron, a once-daily GLP-1 pill from Eli Lilly that the FDA approved on April 1, 2026, for weight loss in adults with obesity, or overweight plus a weight-related condition.12 It is the first GLP-1 pill for weight loss that can be taken at any time of day with or without food.3 This guide covers what Foundayo is, how it works, its doses and trial results, what it costs, and how it compares with Zepbound and the Wegovy pill.
What is Foundayo?
Foundayo is the brand name for orforglipron, a GLP-1 receptor agonist taken by mouth once a day.1 Lilly describes it as a small-molecule, non-peptide drug.3 That sets it apart from semaglutide and tirzepatide, which are peptides. It is approved alongside a reduced-calorie diet and more physical activity to reduce excess body weight and keep it off long term.1
The approved users are adults with obesity, or adults with overweight and at least one weight-related condition.1 Foundayo is not approved for children, and the label says it should not be combined with another GLP-1 drug.1 Lilly made it available from April 9, 2026, through LillyDirect, telehealth providers and retail pharmacies.3
Is Foundayo a GLP‑1?
Yes. The FDA label classifies Foundayo as a GLP-1 receptor agonist.1 It acts on the same receptor as semaglutide but is a different kind of molecule.3
Is Foundayo tirzepatide?
No. Foundayo contains orforglipron, not tirzepatide.1 Tirzepatide is the active ingredient in Zepbound and Mounjaro, which are weekly injections that act on both GIP and GLP-1 receptors.5 Both drugs are made by Lilly, which is a common source of the confusion.
How do you take Foundayo?
Take one Foundayo tablet by mouth once a day, with or without food.1 Swallow it whole; do not break, crush or chew it.1 There is no fasting window and no water limit, which is the main practical difference from the Wegovy pill.36
If you miss a dose, take it as soon as possible, and do not double the next one.1 If you miss 7 or more doses in a row, the label says to restart the dose escalation at a lower dose.1
Two label details matter in daily life. Foundayo can reduce how well oral birth control works, so women using it should switch to a non-oral method or add a barrier method for 30 days after starting and after each dose increase.1 Some drugs that block the liver enzyme CYP3A4 raise orforglipron levels, and with a strong CYP3A4 inhibitor the maximum Foundayo dose is 9 mg.1
Foundayo doses
Foundayo comes in six strengths: 0.8 mg, 2.5 mg, 5.5 mg, 9 mg, 14.5 mg and 17.2 mg.1
Every patient goes from 0.8 mg to 2.5 mg to 5.5 mg, each after at least 30 days.1 Moving to 9 mg, 14.5 mg or 17.2 mg depends on response and tolerability.1 Unlike the Wegovy pill, Foundayo's label lets a person stay at 5.5 mg or another step.16
ATTAIN-1 results: how well does Foundayo work?
In the ATTAIN-1 trial, adults without diabetes on Foundayo 17.2 mg lost an average of 11.1% of body weight at 72 weeks, against 2.1% on placebo.1 On 5.5 mg and 9 mg, average losses were 7.4% and 8.3%.1 The trial enrolled 3,127 adults with obesity, or overweight and a weight-related condition.1
Lilly's own headline is higher: 12.4%, or 27.3 pounds, on the top dose.3 That figure assumes people stayed on treatment for all 72 weeks, while the label's 11.1% counts everyone randomized.13 On the top dose, 54.5% lost at least 10% of their weight and 18.4% lost at least 20%.1
In a second trial in adults with type 2 diabetes, the top dose produced an average loss of 9.6% against 2.5% on placebo.1
Foundayo side effects
The most common side effects are stomach-related. On the 17.2 mg dose, nausea affected 35%, diarrhea 25%, constipation 24% and vomiting 24%, against 10%, 11%, 9% and 4% on placebo.1 Across both trials, 8% of people on Foundayo stopped because of side effects, compared with 3% on placebo.1 Foundayo carries the same boxed warning about thyroid C-cell tumors as other GLP-1 drugs.1
How much does Foundayo cost?
Foundayo costs $149 a month at 0.8 mg and 2.5 mg, $199 at 5.5 mg and $299 at 9 mg, 14.5 mg and 17.2 mg at Lilly's self-pay prices, checked October 7, 2026.4 Two of those are temporary: Lilly has cut 2.5 mg to $149 and 5.5 mg to $199 through December 31, 2026, from regular prices of $199 and $299.4 The $299 price on 14.5 mg and 17.2 mg depends on refilling on time; the regular price for those doses is $349.4

Foundayo vs Zepbound
Zepbound produced larger average weight loss than Foundayo, but the two have not been compared in a head-to-head trial. In separate 72-week trials, Zepbound 15 mg led to an average loss of 20.9% against 3.1% on placebo, and Foundayo 17.2 mg led to 11.1% against 2.1%.15 The trials enrolled different people, so the gap is a guide, not a precise measure.

Orforglipron vs tirzepatide
Orforglipron is a small-molecule tablet that acts on the GLP-1 receptor alone.13 Tirzepatide is a peptide injection that acts on both GIP and GLP-1 receptors.5 In their pivotal 72-week trials, tirzepatide's top dose produced roughly twice the average weight loss.15 For tirzepatide in more depth, see what is Zepbound.
Foundayo vs Wegovy
Foundayo and the Wegovy pill are both daily GLP-1 tablets, but the Wegovy pill produced slightly more weight loss on its label while Foundayo is easier to take. The Wegovy 25 mg tablet led to an average loss of 13.6% at 64 weeks, against Foundayo's 11.1% at 72 weeks on the top dose.16 The Wegovy pill must be taken in the morning on an empty stomach with a 30-minute wait; Foundayo has no food or water rules.16
