Is Wegovy an Appetite Suppressant? What the Lost Weight Is Made Of, From STEP 1 and STEP 6

The FDA label for Wegovy (semaglutide), June 2026, says its effects are "likely mediated by affecting appetite": semaglutide decreases calorie intake and delays gastric emptying.1 Fat mass loss is greater than lean mass loss, but in the STEP 1 trial 40% of the weight adults with obesity lost on semaglutide 2.4 mg over 68 weeks was lean mass.12
Is Wegovy an appetite suppressant?
Yes, as far as the FDA label describes it. Wegovy (semaglutide) is a GLP-1 analogue with 94% sequence homology to human GLP-1, and the GLP-1 receptor is present in brain regions that regulate appetite (label, June 2026).1 The label says semaglutide decreases calorie intake and that its effects are "likely mediated by affecting appetite".1 Semaglutide also delays gastric emptying; the American Society of Anesthesiologists' 2023 guidance table puts the delay at 60 minutes, with a half-life of about one week.13

Does Wegovy burn fat or just suppress appetite?
The weight lost is more fat than lean mass, but lean mass falls too. The FDA label for Wegovy (semaglutide), June 2026, says semaglutide "lowers body weight with greater fat mass loss than lean mass loss".1 In the STEP 1 trial, 40% of the reduction in body weight in the semaglutide 2.4 mg group over 68 weeks was due to a reduction in lean mass: 6.92 kg, or 13.2% of the average baseline lean mass of 52.4 kg (September 2024 review of the trial's body-composition data).2
Visceral fat, the fat around the organs, was measured by CT in a subset of the 401 East Asian adults with or without type 2 diabetes in STEP 6: abdominal visceral fat area fell 40.0% on semaglutide 2.4 mg versus 6.9% on placebo at week 68 (Lancet Diabetes & Endocrinology, February 2022).4
The table draws on the September 2024 review of STEP 1, the STEP 6 paper (February 2022) and the Wegovy prescribing information (June 2026); the STEP 6 row comes from the CT subset, and the waist row counts everyone who started.245

How quickly does Wegovy suppress appetite and start weight loss?
No source on this page measures when appetite falls on Wegovy (semaglutide); the sources time the dose steps and the weight lost at 20 weeks. The FDA label (June 2026) raises the dose from 0.25 mg in weeks 1 to 4 to 0.5 mg, 1 mg and 1.7 mg in four-week steps, with the maintenance dose from week 17.1 In STEP 4, the 803 adults with obesity or overweight and no diabetes who completed a 20-week run-in had lost a mean 10.6% of body weight (JAMA, April 2021).6
The label's schedule is 0.5 mg for weeks 5 to 8, 1 mg for weeks 9 to 12 and 1.7 mg for weeks 13 to 16.1 The STEP 4 run-in was 16 weeks of escalation, then 4 weeks at 2.4 mg; 902 participants started it and 803 completed it, and the 10.6% figure is the mean among those who completed it.6 In STEP 1 (Study 2 in the label), 1,961 adults with obesity or overweight and no diabetes lost 14.9% of body weight at week 68 on Wegovy 2.4 mg versus 2.4% on placebo, a difference of -12.4 percentage points (95% CI -13.3 to -11.6), counting everyone who started (FDA label, June 2026).1
